Your local PCN is still available to support you at this time. We are pleased to continue to offer services but will be doing so differently than before.  All Chronic Disease Management services which were based out of the Recreation Center are now temporarily being offered over the phone.  The PCN will contact patients with scheduled appointments in advance to prepare for this adjusted service.  Please feel free to contact us if you have questions or concerns so we can support you and your families during this challenging time.
