    The Camrose Primary Care Network (PCN) exists to meet the primary care healthcare needs of Camrose and surrounding area through collaborative and comprehensive team based patient care. 

    Background Info:

    The successful candidate will work 5 days (38.75 hours) per week with specific days of the week supporting seniors care delivery at associated continuing care facilities and the Camrose PCN Geriatrics Assessment Program.  The NP is required to practice independently with a high level of confidence and must be capable of effectively collaborating with clients, physicians, and external stakeholders. The NP may be assigned a variety of clinic locations which may change from time to time based on physician support needs, residents care needs and utilization of NP resources. The NP must be physically capable of performing duties related to this position; including assisting residents with physical and mental challenges during faciity visits, lifting of clinical equipment and teaching materials.

    Position Summary

    The PCN Nurse Practitioner will work with multidisciplinary teams to provide a patient centered approach using appropriate evidence-based strategies. The NP will be focused on supporting the delivery of senior’s health care in the local associated continuing care facilities which is inclusive of DSL4, DSL4D, LTC and the Camrose PCN Geriatrics Assessment Program.

    The NP will support geriatric populations and program priorities by:

    Supporting attachment to Camrose PCN Primary Care Physicians.
    Increasing access to primary health care for geriatric and vulnerable populations in the community (e.g., after hours, weekends).
    Supporting transitions in care for geriatric and vulnerable populations from hospital to home to hospital.
    Coordinating and facilitating care for patients with other health care providers, agencies, and community resources.

    Key Responsibilities

    Include, but are not limited to the following:


    Provides high-quality evidence-based health care according to CARNA regulations to the identified practice population.
    Participates in the development and delivery of interdisciplinary, evidence-based health promotion programs to address educational and health needs of patients.
    Acts as a nursing resource to other multidisciplinary team members, healthcare institutions and community agencies or programs.

    Clinical Service Provision:

    Provides individualized health care services to patients, specifically geriatric population, using current advanced knowledge in the diagnosis and treatment of disease by performing advanced, focused and/or comprehensive health assessments and prescribing medication according to CARNA regulations.
    Orders, interprets, and performs diagnostic tests.
    Provides immunization, opportunistic screening, and treatment of communicable disease.
    Consults with Physicians for complex care needs.


    Acts as an advocate, resource, and role model for the provision of competent, holistic, and ethical care.
    Promotes excellence in clinical practice and patient safety through the application of clinical expertise and principles of continuous quality improvement.
    Provides leadership in the development, implementation, and evaluation of evidence-informed practice.
    Leads and participates in quality improvement and risk management activities.

    Communication/Professional Relationships:

    Facilitates communication/information sharing between PCN physicians, AHS health professionals, and/or community agency service providers to promote continuity and consistency in patient care.


    Formal Education

    Nurse Practitioner currently enrolled with CARNA extended practice roster.
    In accordance with the Health Professions Act, the NP must clearly identify any work restrictions to the NP’s license.
    NP must be licensed to work with the family/all age’s patient population or Geriatrics focus would be preferred.
    Current BCLS and CPR certification required.


    Minimum of 3 years direct experience in a community-based setting with an emphasis on primary care services. Other nursing experience will be considered.
    Experience in rural settings and working with vulnerable populations preferred.
    Demonstrated commitment to interdisciplinary teamwork.


    Knowledge and experience in Dementia Care and related behaviors is an asset.
    An understanding of Harm Reduction, Population Health principles and health behavior change methods (e.g., Motivational Interviewing).
    Must be comfortable accessing multiple Electronic Medical Records and other data bases such as NetCare as well as Microsoft Office.
    Ability to be self-directed member of a multidisciplinary team who provides holistic patient care based on advanced clinical knowledge and skills using independent and collaborative problem solving and decision-making techniques.
    Demonstrates good organizational skills in coordinating plan of care for individuals with other team members.
    Reliable and dependable.
    Excellent verbal, written and interpersonal communication skills.
    Ability to safeguard organisational information as private and confidential.
