    Behavioral Health Consultant (BHC) / Social Worker

    Location: Camrose, Alberta

    About CPCN

    The Camrose Primary Care Network (PCN) exists to meet the primary care healthcare needs of Camrose and surrounding area through collaborative and comprehensive team based patient care. 

    The Camrose Primary Care Network (PCN) seeks to fill a fulltime permanent (1.0FTE) Behavioral Health Consultant (BHC)/Social Worker position. This role requires an experienced BHC/Social Worker looking to work to their full scope of practice and, in collaboration with PCN family physicians and the multidisciplinary team, work as a primary care team member to:

    • Provide emotional and social support to individuals and families attached to PCN family physicians;
    • Assist people with social, psychological and/or financial matters that interfere with reaching health goals;
    • Provide information and assistance to people needing access to appropriate community support; and
    • Provide consult and support to individuals as they engage in positive health change activities and when needs extend beyond the medical focus.

    Qualifications:

    • MSW or BSW from an accredited clinical social work program and proof of current registration in good standing.
    • Additional courses or certifications specific to the practice environment and clinical population may be required; and
    • Knowledge and experience working with Microsoft Word, Excel, PowerPoint, Outlook and utilizing an Electronic Medical Record (EMR) and NetCare is an asset; and
    • Current BCLS-HCP certification.

    Experience:

    • A minimum of 5 years’ experience in providing assessment and therapy in a community-based primary care setting including experience in individual and family counselling.
    • Training and experience in mental and behavioral health assessment, care planning, and implementation of evidence-based interventions.
    • Experience working in a collaborative, consultative manner with a varied multidisciplinary team including family physicians.
    • Flexibility and an ability to provide service through brief consultative contacts (30min or less) within a continually changing healthcare environment; and
    • Knowledge of Camrose and area addiction, mental health, and community-based programming supports is an asset.

    This position reports to the Camrose PCN Senior Manager.
