Patient and Family Advisor

The Camrose Primary Care Network believes in the importance of listening to patients and families. Meaningful conversations help us practice what we value and increase our health system’s safety, transparency, and accountability to our residents.
 
At this time, the Camrose PCN is seeking Patient and Family Advisors: people who have personal experience with our services and want to give input, perspective, and engagement to help us give the best care possible.

Registered Dietitian

1 Permanent Registered Dietitian - (0.6 – 1 FTE)
Within the Camrose Primary Care Network (PCN), Family Physicians work together with other healthcare professionals, such as Registered Nurses, Dietitians and Pharmacists to provide enhanced care to patients of the Camrose PCN catchment area.
 
Position Information:
The Registered Dietitian (RD) will work with our multidisciplinary team by assessing, monitoring and reassessing patients in a primary care setting, ensuring that patients receive appropriate and optimal nutrition care. The RD will also counsel individuals and groups within our centralized Camrose PCN Risk Reduction Program and other programs such as Obstetrics, Geriatrics and Palliative and End of Life Care as needed.

The RD will identify priorities, strategies and interventions in order to contribute to patients' individualized care plans. The RD will help patients manage their chronic illness through nutrition interventions. In addition, the RD will coordinate and refer to appropriate group education sessions on various nutrition topics.
 
The RD will participate in professional development and educational activities, including providing clinical education. Some evening hours and travel between sites may be required.  For further information and qualifications please go to www.camrosepcn.com
