• Palliative & End of Life Care Navigator Program

    Today is World Hospice and Palliative Care Day. We are fortunate in Camrose and area because we have the Hospice Society of Camrose and District that provides many valuable services. We are deeply grateful for the work that they do and we are proud to partner with them throughout our programs. 

    We are also using today to talk about a program that we've been working on for nearly a year: the Palliative & End of Life Care Navigator Program. Don't let the name frighten you. Palliative care isn't just for those who are in the end stages of their lives, it's for anyone who has a life-limiting or chronic condition and could benefit from care. It includes supports from the Camrose PCN, Alberta Health Services, healthcare providers, the Hospice Society of Camrose & District, and others. 

    You meet with the Navigator as needed and she helps you access the supports within the healthcare system and the community that work for you. Then, she makes sure that your family doctor and other healthcare providers are up to date with your care plan. You won't have to explain your care over and over again. The program makes sure that your life-limiting illness impacts your quality of life as little as possible.  

    Anyone with a life-limiting illness can benefit from this program and it's easy to access. You can self refer by calling our office and asking for the Palliative & End of Life Care Navigator at (780) 608-4927 extension 2000. Or you can talk to your family doctor for a referral.
