    Transition Nurse

    Position Summary:

    Is a newly designed role intended to support the safe, timely and consistent transitions of patients from hospital to community. Functioning as the key point of contact for transition planning between patient, hospital, and community care teams.  The Transition Nurse’s (TN) coordinated plan is focused on successful patient transition and optimizing communication with community care providers.

    Working collaboratively with an integrated interdisciplinary care team and community partners, the (TN) helps support patients following hospital discharge from St. Mary’s Hospital and Daysland Hospital. A primary goal of this patient centered service is to overcome potential challenges and enable seamless transitions back into the home community setting.  The (TN) understands flow and capacity challenges within hospital and community sector and possesses a strong knowledge of patient transition/discharge care plans.  This extended support service will further increase patient and family member confidence to safely manage their health and care at home.

    Qualifications:

    - Bachelor of Science in Nursing (preferred) or a diploma RN with associated experience or related degree with relevant experience will be considered. 
    - Current registration in good standing with regulatory body. 
    - The ideal candidate will have a minimum five years recent experience in acute clinical setting (Medical, Gynecology and Orthopedic surgery knowledge an asset). 
    - Knowledge of the impact of social determinants of health on transition planning (i.e. length of stay, patient flow etc.). 
    - Working knowledge of hospital and community regulation impacting transition planning (HPA, OIPP, MHA, PHA, HIA etc.); as well as local community resources.
    - The role requires a constructive, empathetic, and respectful approach to interactions with both patients and colleagues.

    Conditions of Employment:

    - Security; proof of clean criminal record and intervention check is required.
    - Copy of related degree/diploma and proof of current registration.
    - This position may work remotely or from a designated location as it supports the care of discharged patients from St. Mary’s Hospital Camrose & Daysland Hospital.
    - This position is out of scope (non-union), part-time and a 1 year temporary, is benefits eligible.

    Salary:  Remuneration based on education and experience with a range of $36.86 - $48.37/hour

    Reporting Structure:  Direct – Clinical Manager
